Summary:
Defines the information and procedures for obtaining and recording information for the birth worksheet and separate records after a hospital or institutional birth. Generally speaking, this bill removes some information from the birth worksheet (such as race, level of education, number of prenatal visits, etc.) and includes it in separate records.
